Q: Hcg 1500 and not even a sac on ultra sound

Hello everyone. I am at what I believe to be 3 weeks 4 days post conception day. I did not get pregnant in the normal ovulation window and instead got pregnant the first couple days after. I had an ultra. Sound done yesterday and it did not even show a sac, but my blood levels were 1500. Does anyone have any idea for this is normal? They did this internal sonogram due to a kidney transplant I had 1 year ago and wanted to make sure everything looked okay inside uterus.

The 21st I was at 1500hcg and 46 hours later I was 2400 and the doctors concerned I didn't double? Thoughts?

1500 is the minimum point for ultrasound to detect a sac. Depending on what type of equipment they used, who scanned you ect ect it still may be too early to tell.

Some ERs will do a transvaginal scan with a portable or handheld ultrasound machine: while convenient, the technology just isn't there on those machines to see such small details. Even right at 1500 a normal high quality machine may not see anything....there are so many variables with ultrasound: uterine position, your weight (obese is more difficult, even vaginally!), intestines blocking the view, and the list goes on and on!!

Usually the best time to start seeing things is around 6 weeks 5 days from the first day of your last period. So give it some time and hopefully everything will work out OK! Congrats!

No prob. The American Pregnancy Association also says:

A transvaginal ultrasound should be able to show at least a gestational sac once the hCG levels have reached between 1,000 - 2,000mIU/ml. Because levels can differentiate so much and conception dating can be wrong, a diagnosis should not be made by ultrasound findings until the hCG level has reached at least 2,000.

So, chin up! You aren't at 2,000IU/ml yet.

Thank you. I had just read so much about people at least seeing the sac by that time and the ultra sound tech hadn't seen anything. I am getting blood drawn again tiday as it is 48 hours since last blood draw.

You said 3 weeks 5 days since conception, but normal HcG charts are usually based off of LMP (last menstrual period), not conception. According to the American Pregnancy Association it is normal for a woman who is 5 weeks past her last menstrual period to have an HCG level of 18 - 7,340 mIU/ml and it's normal for a woman
6 weeks past LMP to have a level of 1,080 - 56,500 mIU/ml. Since you are between 5-6 weeks pregnant, your levels are normal, and you are still very early to see anything. But you should start seeing something very very soon. At your 7 week appointment (based off of LMP, not conception) you should be seeing the baby, and the heartbeat on the screen :). Goodluck!

You are more likely 6 weeks pregnant by the dates you have given.

oh my goodness lol let me know how ot does go when you do get your ultrasound again and if you do end up having multiples!

Thank you for your response... Def makes me feel a little better. I expected my menstral c may 9th or 10th. Ten days ago my hcg levels were 35 and then went to 65 and as of yesterday at 1500

although your hcg levels are extremely high i think its still too early to see anything with an ultrasound im thinking multiples with that hcg level just wait a couple more weeks and if they still dont see anything then look into it further.
