Q: Would you do a D&C under these circumstances?

My good friend found out she was pregnant on October 1st. She started spotting the next week and ended up going to the ER for the bleeding (no cramping) on Friday, 10/12. The ER doc did blood work (HCG @ 2059), and a sonogram that dated the baby at 5 wks 4 days. They also said her cervix was closed and the bleeding was old, but to follow up with her OB/GYN on Monday. So yesterday she went and had more blood drawn and got the results today. Her HCG had gone up from 2059 to 2500 from Friday night to Monday early morning. When she saw her OB today, he said it should've doubled and sent her for another sonogram. They found no heartbeat and the baby had only grown a little - then immediately scheduled a D&C. So, my question is if it's possible that it's just too early to see a heartbeat, or if there really is no chance of a viable pregnancy. The measurements they take at this stage are so small, that it is easy to make mistakes. However, a heartbeat should be seen after week 7. The US she had today should have dated her at 6w4d according to the previous US from october 10th, so chances are that the pregnanct is indeed not viable. The slow rise in hCG is also not a good sign, but all in all I think that it is still possible that she has a viable pregnancy, since the time between the two measurements is very short and the embryo is still very small. I personally wouldn't have a D&C based on this info. There is no health risk if you wait a little longer.
I have had two missed miscarriages, so I do know how it feels, and it is devastating. One thing you really can't have if that you start questioning later whether it was the right decision, or whether there was still some hope. Both times it was made absolutely sure that the embryo did not develop, before I was given the option to undergo a D&C. Best of luck to her..

she should wait it out, did she request a d&c or are they telling her to have one, because there is no reason for it unless its a molar pregnancy and they need to do tests or the woman cannot pass it naturally, or she has personally requested it. they cant make her have one.

Thank you ladies for all your responses! I agree that she should wait it out, but she has the D&C scheduled for Thursday. Maybe (& hopefully) I can figure out a way to bring it up and talk her out of it... I just don't think I would ever forgive myself if I don't try to talk her into waiting it out at least a little bit longer. That, and I know the "what ifs" would never stop going through my head!

I agree. Either see if the m/c happens naturally, and if it doesn't, she should definitely have another ultrasound done in another week or so and see what's there. 5w4d is way too early to see a heartbeat, some don't see it until past the 7th or 8th week.

I had spotting and my first scan was 6+2 weeks ...they saw gestational sack but no baby or heart beat...They asked me to come back after 2 weeks and then my sunshine was measured 8+2 ...exactly as big as he had to be.Nobody never ever checked my HCG levels...Now I`m almost 22 weeks and everything is completely normal....I dont wish to give empty hopes , but I think is better to wait another week or so...

First, at 5w4d a visible heartbeat isn't really possible...so that doctor should know better. I would definitely say to wait on the D&C for at least another week until another ultrasound can be done.

The levels...yeah, they should have gone up more than 2500 over a 3 day period. I can see the doctor's point of view on that aspect. HOWEVER....more bloodwork should be continued and another u/s scheduled before that doctor decides to terminate the pregnancy.

If it were me, I'd want to be 1000% sure the pregnancy was not viable before commiting to a D&C.

I think in that situation my doctor would have had me come back a week later for another ultrasound.
I also think that at that early stage it is better to just wait and naturally miscarry rather than intervene surgically.
She will have to wait 3 months to try again whereas if she miscarried naturally you only need to wait one cycle. That happened to me. No heartbeat. Waited for miscarriage. Got pregnant the next month.

I don't think your question is stupid. I think that the baby kept growing from the 1st ultrasound she had, but then stopped growing shortly after, which is why there wouldn't be a HB and why the HCG had only gone up a little. If she is feeling like it could be viable, then I would repeat an ultrasound in a few days just to be sure. It isn't necessary to have the D&C right away, she can wait a few days to see if the pregnancy is viable. More than likely, she lost the baby, especially if she had an u/s today and they didn't see a heartbeat. So sorry for her loss and hope that she heals and recovers quickly.

(continued).........make sure there's really no chance of it being a viable pregnancy... I never had to go through any of this with my 1st pregnancy or my current pregnancy, so forgive me if my question is stupid :) I just don't understand how HCG levels can increase and the baby grow, and them still be so quick to schedule a D&C...
